Show Title - Artist Submission Form

Thank you for submitting your work to our upcoming show “Show Title”. We’re so excited to see what you want to include.
You may submit up to three pieces and submissions are due by <date due>. If you submit more than one item, please note that the detail fields for subsequent pieces are not “required” by the form, but they are still required. Please be sure to complete the form in order to be considered for this show.
All work will be juried by a selection panel and artists of selected work will be notified by email, using the email provided in the form below.
